  • Recovery Suppport Specialist

  • The Recovery Support Specialist is an advocate who is able, through education or personal experience, to relate to the client’s process of recovery. The Recovery Support Specialist will conduct thorough assessment of assigned clients using SBIRT to determine the extent of substance use/abuse, provide psychoeducation and assist with referrals to appropriate treatment. The Recovery Support Specialist will conduct Recovery Management Check-ups within the framework of the RMCS model to encourage and support the ongoing successful recovery of program participants.

    THE RECOVERY SUPPORT SPECIALIST IS ACCOUNTABLE AND RESPONSIBLE FOR THE FOLLOWING RESULTS:
    • Mentors, coaches and supports clients in their health, wellness and recovery.
    • Conducts toxicology screens with assigned clients.
    • Administers SBIRT assessments with assigned clients.
    • Conducts Recovery Management Check-ups with adult caregivers in person, over the phone and in the community to identify and support progress in recovery.
    • Will conduct toxicology screens with assigned clients, including urinalysis, breathalyzer and hair testing.
    • Responsible for coordinating the provision of services by other community professionals.
    • Models successful recovery strategies
    • Assists individuals with identifying strengths and personal resources to aid in their recovery
    • Provides advocacy and education on treatment barriers and stigma.
    • Helps individuals recognize their capacity for recovery and resilience.
    • Demonstrates an ability to develop a basic level of competency for providing services to clients with substance use disorders and co-occurring disorders.
    • Maintains and updates, on paper or electronically, client case records with required progress notes documenting progress toward service goals and objectives
    • Enters and reviews client and program data in Provider Information Exchange (PIE) database (or comparative alternative) maintained by DCF
    • Attends and participates in monthly coaching calls with model developer.
    • Attends training sessions, workshops, and seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
    • Performs other duties as requested.
    • Required to use own vehicle on company business, or to transport clients.
    • Regular and consistent attendance is an essential function of this position.

    POSITION REQUIREMENTS:
    SBIRT, RMCS
    High School diploma or equivalent required; Bachelor’s Degree in behavioral health field preferred. RSS with less than a Bachelor’s Degree must receive approval by the RMCS model developer.
    Ability to work a flexible schedule.
    Bilingual English-Spanish preferred.
    Volunteer or work experience with adults with serious behavioral health issues.
    Must be well along in own recovery process.
    Proven experience working individually and in a group setting.
    Sensitivity to diverse cultures and language competencies.
    Must have and maintain a valid CT driver’s license. Also, must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.

  • Program Supervisor

  • Responsible for collecting and maintaining client and program information to meet DCF and model developer’s reporting and evaluation requirements. He/she is responsible for managing referrals, intake and case management, and maintaining staff schedules. Directly supervises the work and performance of three full-time MDFR Specialists and two full-time Recovery Support Specialists and the provision of services under the program.

    THE PROGRAM SUPERVISOR IS ACCOUNTABLE AND RESPONSIBLE FOR THE FOLLOWING RESULTS:
    • Acts as liaison to DCF, court system, service providers, staff and families for the SAFE Family Recovery Program.

    • Ensures that all program documentation and reports are completed accurately and efficiently within the scope of service.

    • Receives, reviews and assigns program referrals to the appropriate staff in the department.

    • Attends DCF and court case consultation meetings to discuss program and/or client outcomes.
    • Maintain clinical records in accordance with policies and procedures.

    • Supervises three full-time MDFR Specialists and two full-time Recovery Support Specialists and program operations at CommuniCare service sites.

    • Sets and maintains staff schedule for SAFE Family Recovery Program.

    • Meets with program staff weekly for individual and/or group supervision to ensure support of staff and adherence to program scope of service.

    • Enters and reviews client and program data in Provider Information Exchange (PIE) database (or comparative alternative) maintained by DCF.

    • Consults with the CommuniCare Director (and the QA team) to define work flow, program targets, performance measures and protocols for SAFE Family Recovery Program.

    • Attends training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.

    • Obtains certification in SBIRT, MDFR, RMCS and Hair Testing Collection as part of program orientation and initial training.

    • Required to use own vehicle on company business

    POSITION REQUIREMENTS:
    Licensed with a Master’s degree in behavioral health field (preference for LMFT/LCSW) and minimum of three years related experience delivering clinical and recovery support services.
    Minimum of two years experience in a supervisory role required. Experience supervising staff in a remote work setting preferred.
    At least one year of experience working with culturally diverse populations.
    Must have and maintain a valid CT driver’s license. Also, must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.

  • MULTI-DIMENSIONAL FAMILY RECOVERY SPECIALIST (MDFR)

  • The MDFR specialist will participate on the Safe Family Recovery Team.

    THE MDFR SPECIALIST IS ACCOUNTABLE AND RESPONSIBLE FOR THE FOLLOWING RESULTS:
    • Delivers MDFR services in client homes and community locations
    • Participates in DCF case consultation and service planning meetings
    • Maintains ongoing and regular communication with referring DCF team.
    • Contributes to the substance use assessment of clients referred to program and makes recommendations on levels of care.
    • May conduct toxicology screens with assigned clients, including urinalysis, breathalyzer and hair testing.
    • Facilitates engagement and retention in substance use and mental health treatment for parents and caregivers
    • Provides access to developmental, educational, and medical services for the children.
    • Enhance parent’s involvement in their children’s well-being.
    • Promote emotional attachment and bonding between parents and their children.
    • Improve parenting skills including co-parenting and parental teamwork.
    • Reinforce skills and competencies parents learn in substance use or mental health counseling.
    • Help repair relationships between the parent(s) and other family members.
    • Increase involvement of other positive family members in the children’s lives.
    • Improve parent and family communication, conflict resolution, and problem-solving skills.
    • Collaborate with other department staff to ensure linkage to community resources and services.
    • Serve as liaison with outside agencies.
    • Documentation of program activities in client records.
    • Enter client and program data into Provider Information Exchange (PIE).
    • Attends meetings in the community as representative of program and agency as assigned.
    • Attends training sessions, workshops, and seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
    • Perform any other department or agency related duties or special projects as directed.
    • Required to use own vehicle on company business, or to transport clients.

    POSITION REQUIREMENTS:
    MDFR
    Bachelor’s Degree in behavioral health field required.
    Ability to work a flexible schedule.
    Bilingual English-Spanish preferred.

    Knowledge of behavioral health issues for children and families and of home-based services preferred.
